​We now have Michigan state tax forms and Federal 1040 tax forms! These are the only forms we will have. Other resources can be found here

​What to Bring: 

  • Photo Identification
  • Social Security or Individual Taxpayer ID Number (ITIN) for each family member
  • Birth dates for each family member
  • Copy of last year's federal and state returns (Must have these to complete taxes!)
  • Bank routing & account numbers to direct deposit refund
  • Childcare expenses, provider name, address & tax ID
  • W-2s, 1099s, 1098s & other forms of income
  • 1095-A, B, or C, Healthcare Statements
  • Health Insurance Exemption Certificate, if received 
  • Heat Bill
  • Landlord Info
  • Property Tax Bills
